TCEQ Criminal Background Rule RG-521
By signing this form, I acknowledge Donald Godfrey (C2 Backflow Services, LLC) notified me of the following:
  • the potential ineligibility of an individual who has been convicted of an offense to be issued an occupational license by the Texas Commission on Environmental Quality (TCEQ) upon completion of the educational program;
  • the current TCEQ Criminal Conviction Guidelines for Occupational Licensing, which describes the process by which the TCEQ's Executive Director determines whether a criminal conviction:
    • renders a prospective applicant an unsuitable candidate for an occupational license;
    • warrants the denial of a renewal application for an existing license; or
    • warrants revocation or suspension of a license previously granted.
  • the right to request a criminal history evaluation from the TCEQ under TexasOccupations Code Section 53.102; and
  • that the TCEQ may consider an individual to have been convicted of an offense forthe purpose of denying, suspending or revoking a license under circumstancesdescribed in Title 30 Texas Administrative Code Section 30.33.
